Community Support Network

The mission of the Community Support Network (CSN) is to meet the mental health needs of individuals with chronic and severe mental illness residing in Springfield. CSN utilizes the service delivery model of Community Support Team. This model combines clinical and case management services, working to help clients manage their physical and mental health with the ultimate goal of recovery.

Population Served

CSN serves clients who are age 18 and over, reside in Springfield, have a diagnosis of a severe and persistent mental illness, have difficulty functioning independently in the community, have had multiple psychiatric hospitalizations and have Medicaid.  CSN reserves the right to determine eligibility based on clinical and medical necessity.

Services Include:

• Case management
• Psychosocial groups
• Medication training, delivery and monitoring
• Training in daily living skills
• Education regarding symptom management
• Linkages to other health care providers
• Advocacy
• Referrals for  Supported Employment services
• Financial case management/Representative payeeship
• Assistance in obtaining stable housing
• Psycho-educational multi-family group
• Crisis intervention
• Access to help 24-hours a day, 7 days a week

CSN provides support services to residents of Hope Springs apartments, a permanent supportive housing program. One does not need to live at Hope Springs in order to receive services from CSN.
