At SIU Medicine, we are committed to upholding the highest standards of professionalism and service. While we strive to meet and exceed your expectations, we understand that there may be times when a team member or learner does not meet these standards. To address any concerns or complaints you may have, we invite you to submit your feedback anonymously. 

We will respond to all submissions to the best of our ability. If you would like to receive personal feedback on your concern, please provide your contact information when submitting your complaint.
